Description of action
SSA-System helps you resolve undesirable incidents systematically and learn from them so as to avoid repeating the same or similar incidents in future. Undesirable incidents generally have more than one cause. The causes are determined using the app and the triggers identified are then mitigated with effective measures. The purpose of resolving incidents is not to assign blame.

This is where the incident investigation can be transmitted to the responsible person, along with a deadline for implementation. Define the "type of incident" (occupational accident, non-occupational accident, material damage, near accident) and fill in the corresponding fields.
Description
This is where the situation before the event, the course of event and the situation after the event can be described. Make sure all relevant facts are included and documented. It is also an opportunity to provide pictures.
Cause identification
This is where the various hazards, forces, unsafe conditions and unsafe actions are recorded using a checklist, from which the incident's causes are determined. Alternatively, causes can also be recorded freely without using a checklist.
Measures
Measures are taken to prevent repeat occurrences of the same incident. Always check whether measures have been recorded and implemented – and whether they are actually proving effective.
